All of the above and more... Ankh-Morpork is based on every city that ever had a river through it. Since cities tend to be based around rivers or other watery elements, that includes most of them: London, Paris, Rome, Prague, etc.

Indeed. I think I remember hearing that the only capital city (possibly even major city) in Europe that is not either on a major river or by the sea, is Madrid.

Madrid does have a river but it is very small, when I can get photobucket working ill add a picture.
The original capital however was Toledo, but the legend goes that one of the Spanish kings changed the capital so that his queens hair wouldn't keep going frizzy!
