off the top of my head there were a couple of incidents playing cricket. Both were pretty dumb in that I ended up pretty bashed up, once when facing a very mediocre slow bowler while batting I managed to edge the ball so that it hit me just over my right eye. Another occasion while in the field the ball went past me at speed and too far away for me to get to but not wanting to get abused by my teammate for not trying I dived towards it and ended up landing on my shoulder and compacting the rotator cuff

3 years ago I was playing fetch with my dog he dropped on the top of the patio steps and I bent over to pick his ball up and lost my balance and fell 3 feet onto my back and I both squashed and cracked my L1 vertebra

and had to have 6 weeks off work and 3 months wearing a almost full back brace that
supported me from collarbone to pelvis

[Just some background for the non gamblers here:

I work in a casino and one of the rules of roulette is that the ball must make a minimum of three revolutions of the roulette wheel for it to qualify as a legitimate spin. Also, any foreign object falling into the wheel during the spin disqualifies it from being legitimate (certain bets and transactions during the game are placed on the rim of the wheel). It's important in the event of a 'no spin' to grab the ball in order to prevent it from falling into a number and thus preventing people, with a lot of chips on it, claiming for payment]

Being inexperienced, he just froze in horror as the ball slowly bobbled around the wheel, rather than announcing loudly "NO SPIN" and grabbing the ball before it dropped in a number. I tried telling him to grab the ball a couple of times but was met only with a look of blank, terrified, non-comprehension. As a result it was left to me (and because I didn't have time to climb down off the high-chair), I leant over to grasp the ball. Unfortunately, a combination of physics and gravity caused me to overbalance and fall head-first into the wheel myself. Fortunately, the only thing hurt was my pride and the sides of the customers laughing at my predicament.

For a split second, you thought you saw Death studying a Life Timer very carefully, before he shook his head and replaced it into a crevice in his robe and disappearing ...
Arrgh! Your head hit the table as you fell face down into the roulette wheel. The crash echoed around the room, to notify EVERYONE who hadn't already been aware of the event, as to the disaster taking place.
The hapless trainee still stood frozen to the spot and unable to help.
As you picked yourself up from the table - dazed and bruised - you noticed that you couldn't breathe. 'Nnmph! Nnmph!' you gasped, all the while trying to kid yourself that things were not really as bad as they appeared ... (They were!) ...
As you hurriedly eyed the roulette wheel for the ball, to see if anyone could claim a 'win', your grunting noises became louder and your breathing more desperate.
'Nnmph! NNMPH!' and with a deep breath, you blew your nose ..... PING went the roulette ball, as it shot through the air from your right nostril and ricocheted off the broken wheel ....

I've done this as well, in my case one of the staff closed over a glass door that I didn't know existed so that when I turned around to leave I brained myself on the door
